Referrals

We accept self-referrals and referrals from other services.

We will actively be engaging in the community via community events and door-to-door visits.

Additional information

This is a non-clinical service that caters for an entire geography within the Oakwood area in Warrington.

All residents within the Oakwood area will be eligible for the service.

Our Community Health and Wellbeing Workers will be contacting all households directly, if you feel you or a member of your family might benefit from the service please contact us using the details below.

We aim to discover and help with unmet health needs in the community to help to reduce any local health inequalities.
