Description of the Wellbeing Service

The Wellbeing Service offers a full physical & mental health review from our registered nurses, throughout the Halton & St Helens area.


Who can access the service

Patients who are 18 years and above currently on the Severe Mental Illness (S.M.I) register at their GP surgery.


How can people access the service?

The Wellbeing Service, is accessed from a referral from a GP, Community Mental Health Team (CMHT) or Psychiatry Team.


Where is the service offered?

Throughout the Halton & St.Helens area, at their local GP Health Centre or at a patient’s home.
