Providing in-patient care within a community setting, including nursing, medical, therapy and social care interventions for patients who require intermediate care at Newton Community Hospital.

Intermediate care can be provided to people whose short-term needs can be addressed within a limited period of weeks, as part of their overall care pathway. This could include short-term rehabilitation before moving to a lower level or longer term support.

There is a dedicated multidisciplinary team that support both patients and families including nursing, GP, therapy, social work, and mental health with in-reach support from tissue viability, dietician, continence, heart failure and COPD services.

Patients who live in St Helens or have a St Helens GP who meet the admission criteria.

Patients can access the service via their GP or Via referral from hospital if they meet the admission criteria.
