Integrated Community Nursing Teams: District Nursing, Community Matrons, Treatment rooms and OOH District Nursing.
Description of service
Community Nursing services provide 24 hour service, 365 days per year to deliver high quality nursing care and treatments to housebound patients in a variety of setting but mainly within the patients home, this can include those living in residential care homes, extra care housing and sheltered accommodation.
The integrated nursing teams work with GPs, other specialist nurses, Social Services departments, allied health professionals, acute hospitals and all other health care providers to support and maintain and improve the health and wellbeing of the residents of Halton and St Helens.
In most cases, the integrated nursing team is based within a GP practice.
Who can access the service?
Anyone aged 16 and over and who is registered with a GP from Halton and St Helens. The service will visit patients who live within five miles of the town boundaries if they are registered with a GP from either Halton or St Helens.
How can people access the service?
The service can be accessed 24 hours per day. This may be a self referral, or a referral from the GP, other allied health professional, social services and the acute hospital trusts.
Where is the service offered?
We have 127 staff based across St Helens and 94 staff based across Halton working in this service.
In St Helens we have nursing teams based across the borough including The Beeches, Four Acre Health Centre, Newton Community Hospital, Garswood Clinic and Haydock Health Centre.
In Halton, the nursing teams are based in the following locations:
Widnes: Health Care Resource centre, Chapelfields clinic, Peelhouse medical centre and Beaconsfield Road medical centre.
Runcorn: St Pauls Health Centre, Murdishaw Health Centre, Hallwood Health centre and Castlefields Health Centre.
We also have nurses who run our out of hours service based at Fingerpost Health Centre in St Helens and Health Care Resource Centre in Widnes.
