Description of the Community Specialist Rehabilitation Service

The Community Specialist Rehabilitation Service is a part of the Cheshire and Merseyside Rehabilitation Pathway, supporting patients with complex rehabilitation needs on discharge from the Hub (The Walton Centre), Spokes (Ward 5 Broadgreen and Seddon Suite based at St Helens Hospital) and Extended Rehabilitation Units to continue their rehabilitation within the community.

The Community Specialist Rehabilitation Service is a community based team of experienced health professionals including Occupational Therapists, Physiotherapists, Clinical Psychologists, and Vocational Rehabilitation Therapist with the knowledge and skills to provide highly specialist assessments, co-ordination and rehabilitation to patients with complex care needs including those with complex neurological / post critical care and multiple trauma rehabilitation needs.

The Service also works with service users who have a diagnosis of functional/ Non organic disorder and the team have developed an MUS (Medically Unexplained) Protocol to be able to effectively work with the service users and support them from a psychology led approach.

The core focus of the service is to improve the quality of life for people living with the effects of long term disabilities (including acquired brain injury), by supporting them to achieve their optimal cognitive, physical, behavioural, and emotional levels of functioning with the goal of maximizing their re-integration into the community.

The team ensure that service users receive timely community intervention in line with national quality indicators (National Service Framework for Long Term Conditions and BSRM Guidance 2010).

We provide support to case manage complex care patients within the community and liaise with other services and professionals as required.


Who can access the Community Specialist Rehabilitation Service?

Service users over the age of 18 years who have a Knowsley or St Helens GP.

Service Users with Complex trauma/ neurological needs who would benefit from a multi-disciplinary team rehabilitation approach.


How can people access the service?

Any service user registered with a Knowsley and St Helens GP with Complex trauma/ neurological needs who would benefit from a multi-disciplinary team rehabilitation approach.

Age 16 and 17 will be considered on an individual case consideration basis.

We accept referrals from the network and other services including Primary and Secondary Care Services.

Exclusion criteria

Service users under 18 unless agreed under the criteria for individual case considerations.

Service users with no rehabilitation potential or inability to engage at the time following comprehensive assessment.

Service users with active severe mental health issues, drug or alcohol abuse which would interfere with rehabilitation.

Service users with rehabilitation needs that could be met with pre-existing services e.g. Early Supported Discharge Team for Stroke or Learning Disabilities Services.


Where is the service offered?

The Community Specialist Rehabilitation Service can be offered in the service users own home (including care homes), place of work, educational establishment or in specific clinics.
