Community Matrons – St Helens

Description of the Integrated Community Matrons Nursing Service

The Community Matrons provide a complex assessment of patients with long term health conditions to proactively manage those high risk patients with complex long term conditions.

Their aim is to enable patients to self-manage their own health with support and to prevent unnecessary hospital admission and or facilitate earlier discharge from hospital.


Who can access the Community Matrons service?

Patients who are registered with a St Helens GP who require management of long term complex health conditions.


How can people access the service?

Referrals can be made by:

  • General Practitioners
  • Hospital discharge teams
  • Social Services
  • Specialist Nurses/Services
